What New Patients Need To Bring to Their First Appointment

Our team, led by the highly experienced Dr. Vangala, is committed to creating a warm, welcoming environment for you and your child. From your first visit, we aim to build a relationship based on trust, compassion, and comprehensive care tailored to your child's unique needs. What to bring for an appointment:


  • Previous vaccine records of your child.
  • Previous records from other doctors, including specialists.
  • Your child’s current insurance information.
  • Parent/guardian’s driver's license.
  • New patient registration forms.
  • Please note that a parent or legal guardian must be present for your child’s first visit to Global Pediatrics. We will provide authorization for non-parent consent to the treatment form if you would like a relative to bring your child for their subsequent appointments. If a parent or legal guardian cannot make it to the 1st appointment, they need to sign the form in advance.
  • Please check before the visit that the doctor listed as your child’s PCP (for HMO, CHIP, or Medicaid) is a Global Pediatric doctor.


Please arrive 15 minutes before the scheduled appointment time.
